Terms of Service
These Terms of Service govern your access to and use of Fastform. Fastform is a product of Leadev Software Inc. (Calgary, Alberta, Canada; incorporated in 2020).

Agreement to these terms
By accessing or using Fastform, you agree to be bound by these Terms and our Privacy Policy. If you do not agree, do not use Fastform.
Eligibility
You must be at least 13 years old to use Fastform. If you are under the age of majority in your jurisdiction, you may use Fastform only with the consent of a parent or legal guardian.
Accounts and security
You are responsible for:
- Providing accurate account information.
- Maintaining the confidentiality of your credentials.
- All activity that occurs under your account.
We may suspend or terminate accounts that violate these Terms or pose a security risk.
Plans, billing, and refunds
Paid plans are billed in advance and renew automatically unless you cancel before the renewal date. Taxes may apply based on your location.
Refunds are available only within the first 14 days of your initial paid purchase. After that period, or for renewals, fees are non-refundable except where required by law.

Acceptable use
You agree not to:
- Use Fastform for unlawful, deceptive, or harmful activities.
- Upload malware, exploit vulnerabilities, or interfere with service operations.
- Attempt to reverse engineer or access non-public parts of the service.
- Collect or scrape data from Fastform without authorization.
- Misrepresent yourself or your relationship with others.
Customer content and data
You retain ownership of the content you submit to Fastform. You grant us a limited license to host, process, transmit, and display that content solely to provide the service. You are responsible for ensuring you have the rights and permissions needed to submit and use your content.
HIPAA and PHI
Fastform is designed to be HIPAA-compliant by default. If you are a covered entity or business associate under HIPAA, you may use Fastform to process PHI only when a signed Business Associate Agreement (BAA) is in place. Contactlegal@getfastform.comto discuss a BAA. You are responsible for determining whether HIPAA applies to your use.
AI features and outputs
Fastform may provide AI-assisted features that help you generate forms, summarize submissions, or classify data. AI outputs are provided for convenience and may be inaccurate. You are responsible for reviewing and validating outputs before using them in patient care, compliance, or business decisions. Fastform is not a medical device and does not provide clinical, legal, or professional advice.
We do not use Customer Content, including PHI, to train or improve AI models.
Third-party services
Fastform may integrate with third-party services. Your use of those services is subject to their terms and privacy policies, and we are not responsible for third- party services or content.
Intellectual property
Fastform and its software, designs, and content are owned by Leadev Software Inc. and its licensors. We grant you a limited, non-exclusive, non-transferable license to access and use Fastform in accordance with these Terms.
Feedback
If you provide feedback or suggestions, you grant us the right to use them without restriction or compensation.
Beta and experimental features
Some features may be labeled beta or experimental and are provided "as is" without warranties. We may change or discontinue them at any time.
Suspension and termination
We may suspend or terminate your access if you violate these Terms, create risk or legal exposure for Fastform, or if required by law. You may stop using Fastform at any time.
Disclaimers
Fastform is provided "as is" and "as available." To the fullest extent permitted by law, we disclaim all warranties, express or implied, including merchantability, fitness for a particular purpose, and non-infringement.
Limitation of liability
To the maximum extent permitted by law, Leadev Software Inc. will not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of profits, revenues, data, or goodwill. Our aggregate liability for all claims related to Fastform will not exceed the amount you paid to us for the service in the 12 months before the event giving rise to the claim.
Indemnification
You agree to indemnify and hold harmless Leadev Software Inc., its affiliates, officers, directors, employees, and agents from claims arising out of your use of Fastform, your content, or your violation of these Terms.
Governing law
These Terms are governed by the laws of Alberta, Canada, without regard to conflict of laws principles.
Arbitration and class action waiver
Any dispute arising out of or relating to these Terms or Fastform will be resolved by binding arbitration in Calgary, Alberta, under the Arbitration Act (Alberta). The arbitration will be conducted in English by a single arbitrator. You and Leadev Software Inc. waive the right to a jury trial.
All claims must be brought in an individual capacity and not as a plaintiff or class member in any purported class or representative proceeding. This class action waiver does not apply to claims brought in a small claims court where permitted.
Changes to these terms
We may update these Terms from time to time. If we make material changes, we will post the updated Terms and revise the "Last updated" date above.
